Make sure you fill out the form before you do the course (apart from the pass certificate details, lol) and then you can get the course tutor to sign it and the photo. Many a time I've heard of a form sent back saying it has been signed by an incorrect person, only for them to realise that it was ok after all. This can save several weeks (at the moment it is a 3 month wait!), for the form to be turned around again.
Having the tutor sign the form covers your back (as they have to have been cleared to teach the course in the first place), and you are also sure that you have filled out everything correctly.
